Required Forms
Submission of official transcripts from all previously attended institutions is required for admission to Mount Olive College. Military experience can also be evaluated for college credit with submission of the appropriate transcript.

ALL STUDENTS:
Transcript Request Form - for transcripts being sent from a high school or college to Mount Olive College
A high school transcript is required if the applicant has less than 24 semester hours of
transferable college credit.
Request Form for NC GED Transcript - for NC GED transcripts being sent to Mount Olive College
OR
Submit your high school or college transcripts electronically on Docufide.com.
Currently enrolled NC high school students can have their high school transcript electronically sent to MOC through CFNC.
TRADITIONAL STUDENTS:
Please download, print and complete the following forms. Please return to the mailing address as indicated on each form to ensure receipt by the appropriate office.
Immunization Form - for all incoming traditional freshman
Medical Form - for all incoming traditional freshman
Housing Request Form - for future resident students of the Mount Olive location
Dean's Evaluation Form - for transfer resident students to the Mount Olive Traditional College
Standardized test scores (SAT or ACT scores) are required if the applicant is under 21 years of age or if the applicant has less than 27 semester hours of transferable college credit. Scores included on an official high school transcript will be considered official.
